I can’t find the link to submit my claim
If you've completed your appointment with one of our vetted providers, you can click on this claim form link to submit your claim. Please make sure you're logged in to your Opencare account first to be able to access the link!
When you submit your claim, be sure to attach a copy of your dental receipt which shows:
- Your full name
- Appointment date
- Treatment/services rendered (e.g. new patient exam and cleaning)
- The practice or the provider's name
Our team will use that info to validate your claim and eligibility, and we'll get back to you within 3-5 business.
