How do I claim my reward?

To claim your reward after completing an appointment with an Opencare dentist, follow these steps:

1. Log in to your Opencare account.
2. Click on 'Claim your Reward' in the upper right-hand corner and answer the questions on the web form.
3. Attach a copy of your dental receipt showing your full name, date of the appointment, treatment/services rendered, and the practice or provider's name. You may attach an insurance document as an alternative.

Note: To be eligible, you must have completed a new patient exam and any other dental service, such as cleaning.

Our team will review your claim and eligibility and get back to you.
*Offer claims usually take 3-4 business days to review and complete.
