At Opencare, we’re excited to reward you for prioritizing your oral health. If you've recently completed an appointment with an Opencare dentist, you might be eligible for a special reward! Here’s how to make sure you receive it—quickly and easily.

🎁 Step-by-Step Guide to Claim Your Reward

1. Log in to your Opencare account.
2. Click the 'Claim your Reward' button in the upper right-hand corner and answer a few short questions about your appointment
3. Attach a copy of your dental receipt. Your receipt should include:
   • Your full name
   • The date of the appointment
   • The services rendered (e.g., exam, cleaning)
   • The name of the dental practice or provider

> Alternatively, an insurance document that includes the above information may be submitted instead.

Note: To be eligible, you must complete a new patient exam and any other dental service, such as a cleaning.

⏱ What Happens Next?

Once you've submitted your claim:

• Our team will review your claim for completeness and eligibility.

• You can expect to hear back within 3–4 business days.

We’ll notify you as soon as your reward is approved and processed!
