The dental office did not issue me a dental receipt, can I still claim my reward?
The dental office will be able to provide you with a copy of the receipt upon your request. If, for any reason, you are unable to obtain a dental receipt, you can alternatively send us an insurance document (Explanation of Benefits - EOB) to confirm your attendance at the appointment.
Once you have the required documentation, you can proceed to submit your claim!
