If you're wondering whether you're eligible for our Smile Guarantee Program, here’s what you need to know.

✅ Smile Guarantee Program Eligibility Requirements

To ensure fairness for both patients and dental providers, we follow specific guidelines for eligibility. Please note that eligibility is not automatically guaranteed, even if your request is submitted within 30 days of your appointment.

To qualify, all of the following conditions must be met:

• You must complete a New Patient Exam and at least one dental service (such as a cleaning).
  • Note: Consultations alone do not qualify for a gift card.
• You, as the patient, are at least 18 years old on the date of the dental appointment.
• You have not previously claimed an Opencare offer in the past 2 years (even if you used a different email address to create a new Opencare account).
• Your appointment was booked through the Opencare platform: www.opencare.com only. Appointments scheduled by telephone, in person, or by other means do not qualify for a gift card.
• Your appointment was booked using the name on your Opencare member account. If a single account was used to book 2 or more patients, only one patient would be able to claim a reward.
