Eligibility requirements

To ensure fairness for both patients and dental providers, we follow specific eligibility guidelines. To qualify, all of the following conditions must be met:

Requirement What it means
Service completed You completed a New Patient Exam and at least one dental service, such as a cleaning. Consultations or X-rays alone do not qualify.
Timely reporting You reported your experience to our Support team within 30 days of your appointment date.
Proof of co-pay You provided a receipt showing your total co-pay (the amount paid after insurance). Reimbursement covers the co-pay only, up to $100.
Age requirement You were at least 18 years old on the date of the dental appointment.
Frequency You have not claimed an Opencare offer in the past 2 years, regardless of the email address used to create a new account.
Platform booking Your appointment was booked directly through www.opencare.com. Appointments scheduled by telephone, in person, or by other means do not qualify.
Account match The appointment was booked using the name on your Opencare member account.

Eligibility is not automatically guaranteed — even if your request is submitted within 30 days of your appointment. Every claim goes through a formal review.

How the review process works

To maintain the integrity of the Smile Guarantee, Opencare conducts a formal review of every claim. As part of this process, we contact the dental practice to discuss the appointment and verify the details in your report. This gives us a complete understanding of the visit from both the patient's and the provider's perspective before a reimbursement is approved.

Claims that cannot be verified or that do not comply with our Opencare Reward Program Terms will not be eligible for reimbursement.
