To ensure you and our dentists are treated with fairness, we cannot guarantee claims (even within 30 days). To be eligible:

You must complete a new patient exam and any dental service (like cleaning), at minimum.
- Note: Consultations do not qualify for a gift card.
- You, as the patient, are at least 18 years old on the date of the dental appointment.
- You have not previously claimed an Opencare offer in the past 2 years (even if you're using a different email address to create a new Opencare account).
- Your appointment was booked through the Opencare online platform: www.opencare.com only. Appointments scheduled by telephone, in person, or by other means, do not qualify for a gift card.
- Your appointment was booked using the name on your Opencare member account. If a single account was used to book 2 or more patients, only one patient will be able to claim a reward.
