We love celebrating your commitment to great oral health. To qualify for an Opencare gift card reward, every requirement below needs to be met. Here's exactly what we look for.
Reward eligibility requirements
| ✓ | You complete a new patient exam and at least one other dental service (such as a cleaning). Consultations and X-rays do not qualify. |
| ✓ | Your appointment was booked through the Opencare website. Appointments made by phone, in person, or through other means are not eligible. |
| ✓ | You are not using government-funded insurance, such as Medicaid, Medicare, provincial health plans in Canada, or the Canada Dental Care Plan. |
| ✓ | You complete your visit at a qualified dental office registered with Opencare. If your exam and cleaning fall on different dates, both must be completed with the same office. |
| ✓ | You are a new patient at the dental practice and are at least 18 years old on the date of your appointment. |
| ✓ | You have not received an Opencare reward in the past 2 years. |
| ✓ | Your appointment was booked under the same name as your Opencare account. If multiple appointments are booked under one account, only one patient can claim a reward. |
Submit a valid dental receipt
To finish your claim, you'll need to submit a receipt from your dental office. Make sure it clearly shows all four of the following:
| ✓ Date of appointment | ✓ Name of dental office and dentist |
| ✓ Treatment completed | ✓ Patient name |
Tip: Treatment plans, estimates, and appointment confirmations are not accepted as receipts. Be sure your document reflects treatment that has already been completed.

Rewards are handled directly by Opencare. Please don't contact your dental office about your gift card or claim status—they don't have access to this information.
Your offer must be redeemed within six (6) months of your appointment date.
